NCFA Distributor Social 2018 - ADDITIONAL SUPPLIER REGISTRATION

Please only complete this registration if you have already completed your SUPPLIER REGISTRATION and are sending more than 5 attendees to the NCFA Distributor Social. This registration is for addtional attendees over/above the first 5 included in your sponsorship fee. These additional attendees will be charged $25/pp and this cost does not include free drink tickets, but a cash bar will be available.  Thank you for your support!

The NCFA Distributor Social will be held from 5-8pm at the Crowne Plaza Hotel, 5300 Rockside Rd., Independence, OH.  Discounted hotel room rates of $102 are available until 4/17 by contacting the hotel directly at 216/524-0700 and requesting the NCFA Rate.
